Le’Fevre Personal Care’s Community Residential Alternatives (CRA) program provides residential services for those with physical and developmental disabilities. We have extensive experience working with individuals who have pervasive needs including those with medical or behavioral challenges. And we strongly believe that each person can expand his or her life skills and experience life-long learning. LPC provides residential services in two different types of settings:
Community Living Arrangement (CLA)
A CLA is a group home that provides 24/7 assistance to no more that 4 people in each home with home care, hygiene, medication management, doctor’s appointments, running errands and a host of other things that help you live more independently in your community. All of our group homes are licensed by the Department of Community Health (DCH) and regulated by the Department of Behavioral Health and Developmental Disabilities (DBHDD).
Host Home (HH)
Host homes are residential homes for one, or at the most, two individuals with developmental disabilities. In a host home, an individual with a developmental disability lives with a host family which provides 24 hour per day care and oversight. In a host home, individuals needing care become part of a family whether the host home provider is a single person or a married couple.
Host home providers provide for all of the care needed for a particular individual with developmental disabilities. Assistance is provided with all activities of daily living such as bathing, dressing, grooming, and toileting. Further, medication management, meals, and medical transportation are all responsibilities of the host home provider.
What makes Le’Fevre CRA homes unique?
Services provided are based on each individual’s needs and desired outcomes.
All our staff and host home providers have a wealth of experience in working in the developmental disabilities community. They are all well trained and are committed to promoting the agency’s mission. Also, our staff have passed and local and federal background check; they receive annual training in such as CPR and First Aid, Medication supervision and side effects, Crisis Prevention Intervention Skills, and the Individual Support Plan (ISP) just to name a few.
We are committed to providing individuals with community-based services which keep them active and engaged in their community and provide an alternative to facility-based long term care.
CRA offers each individual an opportunity to learn skills that will help them gain greater independence. Three meals a day are provided, and residents assist in cooking and grocery shopping. They also learn to complete personal tasks such as laundry and room cleaning. The staff creates a warm and structured family-like environment that encourages residents to work together. Independence is promoted, but assistance and guidance are provided when needed.
Funding for Community Living Arrangements is provided through the Department of Behavioral Health and Developmental Disabilities (DBHDD) Intake & Evaluation Team. Individuals must be enrolled in the Comprehensive Waiver Program to receive residential services.
